Liability Release AGREEMENT AND RELEASE OF LIABILITY This agreement is between the Client and Old School Pilates, its owners, employees, contractors, affiliated persons and organizations (collectively referred to as “Old School Pilates”). In consideration for fitness instruction including but not limited to Pilates, Yoga, Reiki, and Bodywork I make the following statements and promises: I understand that fitness instruction and bodywork, as with any sport, could lead to physical injury and I personally assume risk of all injury. I understand that all reasonable efforts will be made to ensure my physical safety. I understand that the equipment used in the fitness instruction can be dangerous if used improperly and I will follow the strict instructions of the instructor with respect to all use of the equipment. I agree that I, my heirs, legal representatives and assigns, will not make a claim or initiate any suit against Old School Pilates. for any injury or damage resulting from my participation in fitness instruction. I agree to hold Old School Pilates harmless from any injury I receive from any participation in Pilates, Yoga, Reiki, and Bodywork. If I am pregnant or if I become pregnant, or if I have or suffer a serious injury or ailment, I will discontinue training until I receive written consent from my doctor.

Workout Waiver Old School Pilates Waiver and Release The Old School Pilates group workout is an independent workout guided and assisted by one teacher. All participants must have experience using classical equipment, a personal workout program, be comfortable working out independently with limited guidance, understand safety guidelines, and know how to set up/clean up/reset the equipment. Old School Pilates has the right to require additional private lessons before permitting entrance into group workouts. Old School Pilates has the right to remove a student from group workout. Old School Pilates has the right to cancel class 24 hours in advance if enrollment is less than 2 persons. I agree to be solely responsible for my safety and well being. I understand that Old School Pilates provides limited supervision, instruction, or assistance for the use of the facilities and equipment in a group workout setting. I agree to comply with all rules imposed by Old School Pilates regarding the use of the facilities and equipment. I agree to conduct myself in a controlled and reasonable manner at all times, and to refrain from using any equipment in a manner inconsistent with its intended design and purpose. I understand and acknowledge that the use of exercise equipment involves risk of serious injury, including permanent disability and death. I have carefully read this agreement and fully understand its contents. I agree that I, my heirs, legal representatives and assigns, will not make a claim or initiate any suit against Old School Pilates for any damage or injury resulting from my participation in a workout. I am aware that this is a release of liability between Old School Pilates and myself and I sign it of my own free will. I agree

POLICIES I am aware that there is a 24 hour cancellation policy and that I will be charged the full session fee for all appointments canceled less than 24 hours prior to the scheduled appointment. I understand that if I have not shown up or called within 20 minutes of my scheduled session time, I will be considered a no-show and will be charged the full session fee. All sales are final and Old School Pilates will not issue refunds, however credit transfers may be arranged. Old School Pilates reserves the right to refuse service to anyone. I have carefully read this agreement and fully understand its contents.
